1. Complete the form below for EACH person attending the reunion. 
2. Once you enter SUBMIT, the form will dissappear. If you made a 
    mistake, simply complete the form again. We will call with any   
    questions. 
3. It is important that we have a separate registration form for EACH person attending the reunion (adult or child). For example: If there are 5 people in your household and each will attend the reunion, then we should receive 5 separate registration forms from you.
